What You Bring

• Licensure: Licensed as an Advanced Practice Registered Nurse (APRN) or Physician in Connecticut.
• Certification: Board Certified as a Family Nurse Practitioner (FNP) or Family Practice Physician.
• Experience: At least one year of post-licensure primary care experience or three years of other post-licensure practice experience.
• DEA Registration: Active DEA Controlled Substance Registration.
